Waiver/Release Form For The Canadian Hish School Football Showcase

I. Consent from Parents or Guardians As the parent or legal guardian of a participant in the Canadian High School Football Showcase Combine Series, I fully support and give my consent for his/her involvement in all related conditioning camps and clinic activities.

II. Consent for Use of Images I permit the use of photographs and videos of my child, captured during their participation in the Canadian High School Football Showcase Combine Series, for marketing and promotional activities related to the event.

III. Acknowledgment of Risk and Waiver of Liability I acknowledge the inherent risks involved in participating in a showcase combine series and agree to accept all such risks. I hereby release and hold harmless the Canadian High School Football Showcase Combine Series, along with its staff, volunteers, and affiliates, from any liability for injuries or damages that my child may incur, regardless of whether they result from negligence or any other cause.

IV. Medical Authorization In the event of a medical emergency during an active conditioning camp or clinic, where I cannot be reached, I authorize the Canadian High School Football Showcase Combine Series to provide first aid, secure necessary medical treatment, or hospitalize my child as deemed appropriate. I have provided a list of any allergies, medical conditions, and regular medications my child has, which is enclosed in a ziplock bag with their name, to be handed to the trainer.

Acknowledgment and Receipt By signing below, I confirm that I have read, understood, and agreed to all terms in this document. My signature also verifies that I have received a copy of this agreement for my records.
